Eligible Production Application

Please fill out the below form if you wish for your production to be considered as eligible for the Betty Mitchell Awards. We encourage all applicants to review the rules for the Betty Mitchell Awards - particularly Section 3: Production Eligibility. Please note that the application must be completed and received no later than twenty-eight (28) days prior to the first public performance. Eligible productions must be open to the general public and run for at least nine (9) performances over a minimum of two (2) weekends for our nominating committee to see your show. If your production does not meet this criteria, please fill out the Request for Provision section below.
